• Only one form should be submitted for passengers travelling on the same flight. If your group includes passengers travelling on different flights, a separate form must be submitted for each flight. 
  

This form should only be completed for Group and Individual participants who have registered and paid for the NEO Science Olympiad or Coding Olympiad Grand Final in New York. Unregistered guests and participants who have selected the Flexy Package should not complete this form.
